Once you determine the company
and guaranteed product that best

where you can complete an online
application request for the product

Step
2: Application
and Paramedical Exam
Within 24 hours of your request,
Premium Brand, Inc. will email
or mail (whichever in more convenient)
an application packet containing
the company selected state approved
application and any other forms
needed to complete your application
for insurance (as required). You
will need to complete and sign
these forms and fax or them to
us at 215-572-7192
or mail to

Application forms can be mailed
to you if more convenient. This
is sometimes easier because we
will highlight only necessary
parts of the application.
Within several days of your application
request, a trained medical professional
will contact you to arrange a
brief medical exam. The paramedic
will visit you at a convenient
location -- work or home -- to
conduct a routine, no cost physical
examination. This exam will consist
of blood and urine specimens,
height, weight, and blood pressure
calculations. To protect your
privacy, your exam results will
be sent directly to the insurance
company.

begin their review until all application
questions, all supplemental forms,
and your medical exam have been
received.*

Step 3:
Underwriting
Approval and Policy Delivery
Once the Life Insurance company
receives all requirements stated
above they will review your
application package and medical
exam results. In many cases
they will request doctor(s)
records on the insured prior
to making their decision.
With most major insurance companies,
the final decision on your eligibility
for insurance will take from
four to eight weeks from the
receipt of your completed application,
medical exam and any other required
information.
The greatest delay most often
occurs when the life insurance
company requests additional
health information directly
from your doctor(s) (they are
paid for this information, but
often are slow in responding
to requests). You can help to
speed this process along by
contacting your doctor(s) and
give them permission and sense
of urgency to release your records.
Throughout this underwriting
period we'll provide you with
status reports via email, or
you can request an up-to-date
underwriting status report by
calling us at 800-220-4015.
Once all the information is
received from all the various
sources, the insurance company
decision will be conveyed to
us. We will contact you with
the details of the insurance
company's decision.
If your policy is acceptable,
then you will remit your initial
premium payable to the insurance
company. Coverage will be "in
force" when the insurance
company receives your premium
payment and any required forms
of acceptance.
